Attendees will learn about the historical evolution of gender-affirming care, the current legal/ethical landscapes, culturally-responsive approaches, and key research findings and gaps. Presenters will discuss essential steps in preparing for trans and nonbinary client engagement.

About this course

This 3-hour online workshop provides a practice-oriented overview of gender-affirming care with trans and nonbinary clients. Attendees will learn about the historical evolution of gender-affirming care, the current legal/ethical landscapes, culturally-responsive approaches, and key research findings and gaps (e.g., core competencies, professional guidelines, standards of care). Presenters will also share clinical applications that are essential steps in preparing for trans and nonbinary client engagement, such as self-reflection, intake, letter referral processes, and common clinical questions relating to experiences of trauma and intersectional lived experiences. Information on lifespan issues influencing trans and nonbinary mental health and wellbeing are discussed, including complex clinical scenarios that will support attendees in knowing when to seek consultation, supervision, and navigate interdisciplinary care.

This is a course that is for health and mental health providers (psychologists, licensed marriage and family therapists, licensed clinical social workers, licensed professional counselors, as well as other health care providers or practitioners such as bodyworkers).

  1. Describe three major developments in the evolution of gender-affirming care with trans and nonbinary clients.

  2. Name two strategies for communicating about informed consent when clients seek letters for gender-affirming care.

  3. Identify the current legal and ethical considerations of providing gender-affirming care with trans and nonbinary clients.

Sand Chang, PhD

Dr. Sand Chang (they/them) is a Chinese American nonbinary psychologist, author, and trainer with 20+ years of experience working with individuals and organizations. Dr. Chang’s work is rooted in social justice and body liberation for all, with a special focus on gender affirming care.

Dr. Chang’s writing includes All Parts Welcome: The Queer and Trans Internal Family Systems Workbook (2025), A Clinician's Guide to Gender Affirming Care (2018, 2026), and APA Guidelines for Psychological Practice with Transgender and Gender Nonconforming Clients (2015). Dr. Chang has served as a board member and editor for Trans Bodies, Trans Selves, co-author of WPATH SOC8, co-founder of The Gender Affirming Letter Access Project (The GALAP), member of Harvard Medical School Sexual and Gender Minority Health Equity Initiative Professional Advisory Council, and Chair of the APA Committee on Sexual Orientation and Gender Diversity (CSOGD). Dr. Chang’s clinical work is focused on eating disorders, trans health, and trauma healing through Internal Family Systems, Somatic Experiencing, and EMDR. They are a Lead Trainer with the IFS Institute and Queer and Trans Internal Family Systems (QTIFS). Outside of work, Sand is a dancer, astrologer, and pug enthusiast.

Anneliese Singh, PhD

Dr. Anneliese A. Singh is an internationally renowned researcher and licensed counselor and counseling psychologist whose work centers on racial healing and racial trauma, as well as the resilience, identity development, and other health outcomes among trans and nonbinary adults. Dr. Singh is the author of The Queer and Trans Resilience Workbook and The Racial Healing Handbook, and has co-authored and co-edited books on gender-affirming care (Affirmative Counseling and Psychological Practice with Trans and Nonbinary Clients with Dr. Rafe McCullough and the Clinician’s Guide to Gender-Affirming Care with Dr. Sand C. Chang). Dr. Singh has worked on counseling and psychological practice competencies and guidelines in both the American Psychological Association (APA) and the American Counseling Association (ACA) with trans and nonbinary clients. A past president of the Society of Counseling Psychology in APA, Dr. Singh has received numerous national awards for contributions to research, practice, and advocacy. Dr. Singh is Fellow of APA and ACA.
